Shipping Instructions:     Package your item in original carton if possible. If not, wrap the unit in bubble wrap and place in a box large enough to fill in with styrofoam peanuts. Place a copy of this form inside the box. Put a label on the outside to the above address. Ship by UPS, Mailboxes etc., or US Mail. Also, please insure for equipment value.
